For Japan, April is perhaps the month that most clearly symbolizes the spring and the start of many things. The school year starts in April, the new fiscal year, and many companies hire new employees as well.

Perhaps that season of change is rooted in the fact that April is the month with the most drastic change in the weather. April is the month with the most significant rise in temperature, and especially during the early days of April, the cherry blossoms are in full bloom in many regions of the country.
Tokyo has an average temperature that is around 20 C during the day. Note that because the weather is changing a lot, the temperature tends to alternate between cool and warm spells so it can be a bit tricky to know how to dress. Furthermore, the humidity level also is on the rise with rainy days that become more frequent.

So bottom line if you are planning a visit in April, it's a good idea to bring a mix of light and warm clothes. You might need a jacket during the early hours of the day and be fine to walk around in a t-shirt in the afternoon, and then need that jacket again once the sun sets. As for rain gear, it's not really necessary if you're planning to stay in the city as you can buy cheap ¥500 umbrellas and raincoats at convenience stores located virtually at every street corner.

This is the general weather pattern for most of the main islands except for Hokkaido which is quite colder than the rest of the country. If you are planning to travel to Sapporo for instance, expect weather that is still on the cool side for April (around 10 C during the day and near the freezing point at night).
Of course, the alpine regions are also still quite cold, which is a good thing if you're planning a spring ski trip! You can still enjoy some good snow well into April and even later. The picture above is Yuki no Otani, a famous valley known for having some of the highest levels of snowfall in the country.
On the other extreme, if you are planning on visiting the beautiful southern prefecture of Okinawa, the weather there will be summer-like. In Okinawa, the end of March period is referred to as the umi-hiraki 海開き (opening of the sea) period, which means that you can start swimming in the Ocean! The islands of Okinawa benefit from a warm ocean current called the Kuro-shio, so even though the air is not that hot (daily mean temperature of around 25 C) the Ocean is warm enough to swim in.

Don't expect to see cherry blossoms in Okinawa in April! Their cherry blossom season is much earlier, in late January and early February.
To sum up, the weather in Japan in April is cool to warm for most of the major cities in the country (Tokyo, Osaka, Kyoto, Fukuoka), much colder in Hokkaido (Sapporo) and in the mountains, and summer-like in Okinawa.

As a reference, here is the average high temperature in April for some major cities in Japan from north to south.

Sapporo, Hokkaido: 11.5 C (52.7 F)
Sendai, northern Honshu: 15.0 C (59.0 F)
Tokyo, central Honshu: 19.0 C (66.2 F)
Osaka, southern Honshu: 19.9 C (67.8 F)
Fukuoka, Kyushu: 19.5 C (67.1 F)
Naha, Okinawa: 24.1 C (75.4 F)
